The following forms are required for registration with the GSHL.

USA Hockey Participant Code of Conduct
USA Hockey Consent to Treat/Medical History Form
A copy of the student's school schedule, school ID, or some form of mail showing the address to prove what school the player is districted for.
The forms, hopefully all online soon, need to be turned into your team who will turn them into the registrar. Once the forms are received and validated by the registrar, the player will be added to the team's official roster and will be listed on the webite.
